How to get your free lunch sack…

1. Please print the following form and fill in your information.
2. Mail the completed form along with TWO Proofs of Purchase (per lunch sack) of eligible items from Annie’s or the partner brands to: Annie’s Lunch Sack, 1610 Fifth Street, Berkeley, CA 94710.
3. You must MAIL in your form and Proof of Purchase in order to receive your lunch sack.
4. Eligible products include: any Annie’s Homegrown product, Stonyfield YoKids Organic Yogurt 6-Packs, 4-Packs, or Squeezers, Honest Kids drinks 8-pack carton or 64oz. bottle OR Seventh Generation disinfectant sprays or wipes.
5. Allow 8-10 weeks for delivery.
